Welcome to the Health Information Department at Harrison County Hospital. We maintain records for Harrison County Hospital and its physician-owned practices.

To request records:

Most records are available through our patient portal, YourHealthLink.

Do you need records sent to another provider? Please contact us at 812-738-7885. We are happy to fax your records to other doctors for continuation of your care.

If you need a copy of records for personal use, you can come to the Health Information Department with your photo ID to request. Please note that there are processing fees involved. Photo ID is required for release of records.

Must provide photo ID. There may be a fee included for processing requests.

Need a copy of your Radiology images?

Need a copy of a birth or death certificate?

Please contact the Harrison County Health Department at 812-738-3237 (Vital Records Department)

E-Notifications Requestor Portal

Harrison County Hospital has chosen PatientPing to facilitate the delivery of real-time event notifications for care events occurring at its facilities, as required by CMS’ electronic event notifications Condition of Participation (e-notifications CoP)*.

To request e-notifications for your patient’s care events at Harrison County Hospital, please fill in the required information.

Once you have submitted your information, a PatientPing representative will contact you directly to assist you in enabling real-time, e-notifications from Harrison County Hospital.
